Population(s) Served
Adults and/or children with Intellectual and Developmental Disabilities (I/DD), Autism Spectrum Disorders, or other qualifying support needs who require assistance to actively participate in the community
Settings
Services are provided in person in community-based settings such as recreation centers, libraries, employment sites, volunteer sites, faith communities, and other integrated public settings.
Hours of Services
Services are offered based on individual needs and authorized funding units, typically ranging from part-day to full-day schedules
Days of Services
Services are available Monday through Sunday, including evenings and weekends as requested and approved
Frequency of Services
Determined by the person’s Individual Support Plan (ISP) and authorization from the payer.
Payers and Funding Sources
Medicaid Waiver Programs, State/Local funding, Private Pay, and other approved funding sources.
Fees
Fees are based on the established reimbursement rate per unit as defined by the funding source.
Referral Sources
Case Managers/Service Coordinators, families/guardians, schools, mental health providers, state/local agencies, and self-referral.
Specific Services Offered
Skill development, social engagement, transportation training, community participation, safety awareness, and support to access integrated activities. Services are provided directly by trained staff.
